1. FILL OUT THE ALOHA MEDICAL CANNABIS QUALIFICATION FORM

If you are requesting an appointment for a medical cannabis card through Aloha Medical Services, you must first complete a Qualification Form in order for us to know if you are eligible to receive a Medical Cannabis card.

2. MEET YOUR Qualified Medical Provider (QMP)

If our QMP’s assessment leads to evidence that you are a good candidate for a Medical Cannabis Card will, you will be prompted to make an appointment and meet with your QMP.

3. APPLY CARD ONLINE

Apply for your card online at medicalcannabis.utah.gov/patients/ and notify your QMP your application is awaiting their certification.

4. APPROVES YOUR RECOMMENDATION

Our QMP approves your recommendation.

5. PAY FEES

Return your application and pay application fee of $15.

6. Utah Department of Health (UDOH) REVIEWS APPLICATION

UDOH reviews and approves your application.

7. RECEIVE AND REVIEW YOUR CARD

Receive your medical cannabis card via email and review it for accuracy.

8. PURCHASE MEDICATION AT A UTAH DISPENSARY
